    NOTE: Please prepare your soft/scanned copy of the following online admission requirements:

 

1. FOR NEW / TRANSFEREE STUDENTS:

• Latest Report Card (Form 138)
• Certificate of Good Moral Character (for incoming Grade 3 up to Grade 12 only)
• Clear copy of NSO/PSA Birth Certificate
• Clear 2X2 ID picture of the student with white background (from head to shoulder only)
• Certificate of Completion (for incoming Grade 11)

2. FOR OLD STUDENTS:

• Latest Report Card (Form 138)
• Clear 2X2 ID picture of the student with white background (from head to shoulder only
